After-Hours Care:
If you are an established patient and have an urgent medical concern after clinic hours, please call our answering service at (844) 738-1673. They will contact the provider on call, if appropriate.

Family Practice Appointments
We value your time and schedule appointments to help keep your visit running as smoothly as possible. Please contact our office in advance to schedule an appointment. As a courtesy, we provide appointment reminders approximately two days before your scheduled visit.
If you are unable to keep your appointment, please notify us as soon as possible so we can offer that time to another patient. While we make every effort to stay on schedule, unforeseen emergencies may occasionally result in delays. We appreciate your patience and understanding.
Our providers work together as a team to ensure you receive timely, high-quality care. Whenever possible, we encourage you to see your primary care provider. However, if your provider is unavailable or delayed, you may choose to see another provider in the clinic. If you would like to change your primary care provider, please let our front desk know, and we will be happy to assist you.
Please arrive 15 minutes before your appointment to allow time for check-in and any necessary paperwork. We also ask that you bring your current insurance card and photo identification to every visit so we can verify your information.
