Family Practice Appointments
We know your time is important, so we run our office on an appointment basis. Please contact us in advance so we can schedule an appointment for you. We have an appointment reminder system in place that will call you two days prior to your appointment. If you are unable to keep your appointment, please notify us as early as possible. We make every effort to honor all time commitments, but we may have an occasional emergency which may cause delays. Your patience is greatly appreciated during these times.
Our family of providers function as a group. It is in your best interest to see your provider whenever you come to the Clinic. However, with our group concept, you and your family have the option of seeing one of our other providers should your provider be out of the Clinic or delayed by an emergency. It is also our philosophy that the relationship between you and your provider is one of utmost importance. If you should wish to change to one of the Clinic's other providers, we encourage and support your decision, and ask you to notify the receptionist to make the change.
Please arrive 15 minutes before your appointment to complete the necessary forms and have your insurance card available to be verified at every appointment. Thank you.
