DOT & CDL Physicals

Keeping you safe while you drive.

We are now offering DOT (Department of Transportation) & CDL (Commercial Driver's License) physicals. This type of physical helps determine if a driver is mentally and physically fit to operate a commercial vehicle.

Do I need a DOT Physcial?
You are required to have a DOT physical if:
- You transport hazardous materials
- You operate a vehicle that carries more than 15 people
- You operate a vehicle with a gross combined weight of more than 10,000lbs
- You are paid to operate a vehicle that carries more than 8 people

Setting Up Your DOT/CDL Physical
- Appointment is Required. Call 360-754-6367, press 0 to speak with a receptionist to schedule an appointment with our certified provider, Megan Colburn, ARNP.
- $120 out of pocket payment is expected at the time of service. This exam is not covered with insurance.
- Bring your U.S. Department of Transportation Medical Examination Report Form and make sure to have "Driver's Information" completed before your appointment.
- Bring a list of your prescribed & over the counter medications to include the name of your medication, dose & frequency, diagnosis or reason for taking the medication, who prescribed it, and how long you have been taking it.
- Bring a list of your surgical history to include the type of surgery, date, where it was performed and name of the surgeon.
- Bring a list of illnesses or injuries requiring medical treatment during the past 5 years and or any history of a seizure or a heart condition including the type of illness or injury, dates of treatment, where treated, name of primary person (or facility) that provided the treatment.

 What to Expect During the Physcial
A certified medical examiner will review your medical history, check your vital signs and perform a hands on physical exam that includes but is not limited to:
Vision
Please bring your glasses or contact lenses if you require them to drive.
Hearing
Please bring your hearing aids if you require them to drive.
Blood Pressure
Submit a Urine Sample

At the end of the exam, we will provide you with a medical examiner's certificate when you have met the DOT physical requirements.

Telephone Calls & Rx Refills

Our trained staff will gladly answer your health, billing and other related questions. If you need to speak to your provider or nurse, or have an urgent problem, please inform the receptionist. Urgent calls will be connected immediately or returned as soon as possible. All other calls will be returned as your provider's or nurse's schedule allows.

We have voice mail so that you can leave a detailed message if the person you are trying to reach is unavailable. When we return to our stations we are immediately notified of waiting messages, and will return your call as soon as possible.

In most instances, we believe it is not in your best interest for us to prescribe medicines by phone. If you have not seen your provider for a year, you must do so to have a prescription refilled. Always contact your pharmacy for refills rather than calling the clinic. If needed, the pharmacy will contact us and the request will be processed within 48 to 72 hours excluding weekends. Please do not call the clinic before that time as the refill is in process.
