1. How do I get my records from a previous provider to be added to my chart with TFPC?
You can stop by the clinic and request a "Incoming Records Request" form from the receptionist. On the form you will have the option to add up to 4 different providers. You will need to include the clinic's name, address, contact and fax number. Once completed, the receptionist would be able to fax it to the office you are requesting records from.

You can print off the "Incoming Record Request" form from the "Forms" page on our website. Once you've completed the form, you can either fax it to our office at (360)754-6429, mail it to our clinic at 150 Dennis St. SW Tumwater, WA 98501 or drop it off to our office. Once we receive it we will fax it to the office you are requesting records from.
2. What if I have my records on a CD? How can I get these added to my chart?
We are only able to accept paper copies of medical records. If you have your previous medical records on a CD, you can print the most pertinent documents you feel it is important for us to have as part of your record with our clinic and drop it off with our receptionist. We will have our Medical Records department scan it into your chart.
3. How can I get my records from TFPC?
You can either stop by the clinic and ask a receptionist for the "Outgoing Record Request" or print off the form from our website from the "Forms" page. Once you have completed the form, if you printed it online, you may fax, mail or drop off the form to our office. Once we have received it, it would take 14 business days for our Medical Records department to complete your request.
4. How long does it take for me to get my records once I've requested them?
Up to 14 business days.
5. I want my records emailed to me, is that possible?
Yes! You would need to fill out the "Email Records Request" form on our website from the "Forms" page and email us the request to: [email protected]*Please note that TFPC does not have encrypted email. Requesting your records via unencrypted email carries a degree of risk as there is potential for unauthorized access during electronic transit.
6. How can I get a copy of the images of my xrays?
We only receive the written report of your imaging/xray results. If you would like a copy of the actual image, you will need to contact the facility that did your diagnostic imaging for this request.
