Dear Patient,
This letter is to let you know that Paul Surette, PA-C, has made the decision to leave family practice and will therefore be leaving the clinic in early July. Paul is going to work at an urgent care facility which means unfortunately patients will not be able to follow him for primary care.
We are actively recruiting a provider to replace Paul but at this point we have not found that person and anticipate it may be months before we have a replacement. All of our remaining providers have very full patient panels and are not able to take on additional patients. Dr. Kuo is leaving in the fall and one of our other providers will be out on maternity leave from Oct - Feb. In addition, we cannot promise we will have a male provider, we will hire the best candidate for the position.
Until we find a new provider, we may not have the capacity to continue to see Paul’s as quickly as usual. We will continue to try and accommodate as much as we can, but we need you to understand that we may ask you to go to urgent care if you call for urgent concerns, and follow up appointments may be scheduling farther out than normal. In addition, it may take us more time to respond to non-emergent issues, phone calls/refill requests etc.
If you are concerned this may not meet your medical needs during this time, or you prefer to see a male provider, we encourage you to find a provider outside of TFPC. We have attached a list of local offices taking new patients.
Please check our website periodically to see when we have been able to bring on a new provider. If you need to have your records transferred to another office, you can download a records release form from our website, or ask a receptionist.
